I/We request that the name of our above-named child be registered as a prospective pupil. I/we confirm that payment of the £100 registration fee has been made by bank transfer to: Lloyds Bank, Sort Code: 30-92-63. Account No. 03331173 (Please quote your child's name (surname followed by first name) and 'Reg' to help us identify your Registration Fee payment),​ or I/We have sent a cheque for £100 for the non-returnable registration fee​​. ​Please note, the registration fee will be increasing to £175 from 2 January 2022.​
We are legally obliged to have the full details of all adults who have parental responsibility for a child and ask that you provide this information in the Parent / Legal Guardian boxes above. If only one person is mentioned, please provide a brief explanation.​
I/We understand that the School’s Terms and Conditions will undergo reasonable changes from time to time as circumstances require and will apply in all our dealings with the School. I/We understand also that the School (through the Head, as the person responsible) may obtain, process and hold personal information about our child, including sensitive information such as medical details, and I/we consent to this for the purposes of assessment and, if a place is later offered, in order to safeguard and promote the welfare of our child.
